PRISM is America's first virtual care program dedicated exclusively to stroke survivors. It was developed by neurology experts at Mount Sinai hospital in New York City, one of the worlds leading stroke centers. Many thousands of patients have benefitted from the service, which helps to identify the early signs of stroke recurrence and allows earlier intervention.

The PRISM Experience

Experience peace of mind knowing that you will join thousands of other patients on a program proven to improve long term outcome

Enrollment

You’ll either be setup in the hospital or equipment will be mailed to you at home

Set-Up

You’ll download the PRISM App and you’ll be shown how to use it and take readings with the blood pressure cuff

Connect

You’ll be assigned a dedicated care partner who will help you get connected with rehab specialists and medical care

Measure

Regular Blood Pressure readings and health data are captured through the app and shared with your care partner

Monitor

Your Care partner checks your health data daily and arranges for medical follow up if necessary

Assess

Your Care Partner will perform regular evaluations using validated stroke assessments to measure recovery progress

Share

Information on patient progress is shared monthly with caregivers and providers

Day 7

We’ll connect you with rehab specialists in less than 7 days of you joining

  • Neuroplasticity is highest within the days and weeks following discharge. The earlier you get into rehab, the faster and better you will recover.


Day 30

We’ll make sure that your blood pressure stays under control

  • Stroke survivors report high levels of depression and sleep disruption. Addressing these issues improves wellbeing and long term outcome.


Day 90

We’ll reduce your risk of readmitting to the hospital by up to 50%

  • Elevated blood pressure is a leading cause of secondary stroke - up to 80% of strokes are preventable through blood pressure control


Day 365

We’ll check your mental health and ensure you stay active and sleep well

  • Visits to the hospital are scary and put patients at risk. Secondary strokes are often fatal or severely debilitating.
