PRISM for Mild Stroke

$49 Per Month

  • A mild stroke (sometimes known as a Transient Ischemic Attack or TIA) is a temporary blockage of blood flow to the brain, which usually resolves itself without the need for medical intervention. There are often mild residual symptoms that dont limit daily activities.

If you have just had a mild stroke, it is important that your blood pressure is closely monitored and that you take positive steps to reduce the risk of having a more serious stroke in the coming weeks and months. PRISM for Mild Stroke is a nurse-led health monitoring program focused on blood pressure control, lifestyle coaching and education.

PRISM for Serious Stroke

$99 Per Month

  • A serious stroke (often called ischemic or hemorrhagic) will disrupt blood flow to the brain and require surgery or medical treatment to remove the blockage or fix the rupture. Patients will often have a lengthy stay in the hospital and serious symptoms following stroke.

If you have just had a serious stroke, it is important that your blood pressure is closely monitored and that you receive the right care to keep you out of the hospital and get you back to being you as quickly as possible. PRISM for Serious Stroke is a nurse-led health monitoring program focussed on blood pressure control, care co-ordination and quality of life improvement designed to improve your outcome and lower your risk of readmission.

  • The main goals are to help stroke survivors maximize their recovery potential, reduce the risk of readmission to the hospital and to ensure all patients are connected with high quality rehab services within 7 days of joining the program, if needed.

  • You'll get access to a dedicated care partner, will be provided with an electronic blood pressure cuff and you’ll get access to our PRISM app. The service you get will depend on whether you experienced mild or serious stroke.

  • While PRI does employ staff who are medically trained and licensed, PRISM is not a medical service and does not replace usual medical care. If you are feeling unwell at any time while on the PRISM service you should call a doctor or 9-1-1 if it is an emergency.

  • No. PRISM is a care coordination and monitoring program. We do not provide rehabilitation services such as physiotherapy, occupational therapy or speech therapy. We can however recommend high quality providers in your area offering these services.

  • While we would encourage you to consult your medical team prior to signing up, there is no requirement for their consent or involvement in the program.

  • Yes, PRI takes extensive measures to ensure there is no loss of health data and that only your care partners have access to your data so that they can deliver the service. You can learn more about how we handle data in the PRI privacy policy.

  • You can cancel any time within the first 30 days of purchase for any reason if you are not satisfied with the service. Our full refund policy can be found here

  • PRI employs licensed Physiotherapists and Nurses who have speciality training or knowledge of neurological disease to act as care partners for subscribers to our service.

  • Yes, often family members or caregivers buy the PRISM program for a third party. If you would like to do this, you will enter the patients name and address in the shipping information and provide their email address so we are able to contact them. You can still enter your payment details into our payment form such that the payment comes from your payment method.

  • Absolutely, it is encouraged for you to share contact details of family or other caregivers with your PRI care partner and to involve them in your care plan. If you provide these to us we can share your monthly progress reports with them via email (if you have subscribed to the standard or premium service). Unfortunately we are not able to provide anyone except your PRI care partners with access to your PRISM clinical dashboard.

  • If you would like to join PRISM you can click on the buy now button next to the service level you would like to purchase on the Buy Now page. Once your order has been received you will be sent an email with detailed instructions of how to download our mobile application - once you have downloaded your mobile application you can schedule your first call with a PRI care partner. You will receive your monitoring equipment in the mail within a few days.

  • While we would encourage you to consult your medical team prior to signing up a prescription or referral is not required to participate.

  • No, this is a self-pay service which is not covered by insurance today. PRI are working towards gaining coverage for the service through Medicare and other commercial plans. Once this is available we will inform all members.

  • No, there is only a single monthly fee based on the type of stroke you experienced.

  • Yes, your care partner will help you to create reports that can be shared with your medical team.
