PRISM for Serious Stroke is a nurse-led health monitoring program focused on blood pressure control, care co-ordination and quality of life improvement designed to improve your outcome and lower your risk of readmission. It is intended for patients who have experienced a serious stroke, such as an Ischemic or Hemorraghic Stroke, which required treatment in the hospital.
